Referral Line: (646) 801-1017Silo Psychiatric was among the few facilities in the region to maintain a dedicated treatment track for eschatological delusion — the fixed, unshakeable belief that the world has ended, is ending, or has already been replaced. The program is directed by the facility’s founder and Medical Director.
Standard reality-orientation techniques are of limited use with this presentation. Treatment emphasizes stabilization of routine over correction of the belief.
A fixed daily routine is established before any therapeutic engagement with the delusional content; predictability reduces acute agitation more reliably than confrontation.
Patients are encouraged to render the ending as they experience it — in writing or in the studio — both as clinical record and as a means of externalizing a belief that resists speech.
A single consistent team is assigned per patient for the duration of admission. Familiarity is the active ingredient: a steadying presence when the patient holds that nothing else remains.
Expressive art therapy is a cornerstone of care at Silo, and a required component of the World-End Ideation Program. The facility maintains a dedicated studio where patients work several mornings a week under the supervision of a licensed art therapist.
For patients whose experience resists ordinary language, the canvas often comes first. Much of the work produced here is retained in the clinical archive, and a rotating selection hangs in the studio.
Dr. Tutsia founded Silo Psychiatric in 1982 after more than a decade in regional inpatient care. He built the facility around a single conviction: that for patients whose sense of reality has failed, the most therapeutic thing a clinician can offer is not argument but constancy — a routine that holds, a team that returns, a ground that does not move.
His clinical interest in eschatological delusion — the belief that the world has ended — is the facility’s defining specialty. He directs the World-End Ideation Program personally, and publishes sparingly, preferring, he writes, “to remain with the patients rather than to explain them.”

Silo Psychiatric was established in 1982 as a regional inpatient facility and has grown into a comprehensive behavioral health provider. Our clinicians maintain particular expertise in persistent delusional presentations — including the eschatological, or “world-end,” ideation that our founding director has studied throughout his career. Our approach emphasizes the therapeutic value of routine, structure, and the steadying presence of a consistent care team. Whatever a patient is facing, we work to ensure the ground beneath treatment will hold.
